Please arrive 15 minutes before your scheduled appointment to allow time for any updates to patient files and other necessary paperwork. Please bring the following to every appointment:

•   Photo ID

•   Current insurance card(s)

•   Co-payment(s), if required

•  All your medications, prescription and over the counter

•  Tests results/films that have been requested by us

•  Past medical records you feel are applicable to your health

•   If you have Medicare, your pharmacy insurance drug card
