Mammography Services No Longer Available

Dear Patient,

It has been our pleasure to provide mammography services at Horizon Family Medicine for the past 19 years. Effective June 1st, 2021, mammography services will no longer be available at Horizon Family Medicine. To obtain a copy of your prior mammograms, we request that you complete a medical records release form. This form can be found on our website, mailed to your home, or picked up at the front desk. Prior images can also be requested by other facilities with your consent. Please have all other facilities fax requests to (919) 934-5632.

Breast cancer screenings are an important part of your health care. The physicians of Horizon Family Medicine encourage you to obtain your records as soon as possible to prevent any delays in care.

The Four Oaks office is partnering with a company to provide mobile mammograms on site. If you are a patient of the Four Oaks office, please call (919) 963-3148 for details. We appreciate your patience during this time.
