As you know, our practice, community, and hospital system have grown rapidly over the last several years. With the goal of keeping patient care at the level of excellence you deserve, Horizon is pleased to announce a partnership with the UNC Rex Hospitalist program to oversee care for all adult Horizon Family Medicine patients hospitalized in our local hospital campuses.
Starting November 15, 2015, all adult patients of Horizon Family Medicine hospitalized at the Johnston Health – Clayton facility will be cared for by the UNC Rex Hospitalist Physician team.
On February 1, 2016, he same transition will take place at Johnston Health – Smithfield facility.
As the largest primary care practice in our county this decision was not one that was made lightly. Our providers care deeply about our patients and we want to make sure that our patients’ care across the medical system is as seamless and efficient as possible. To that end, clear communication will be maintained between the UNC Rex hospital physicians and your personal Horizon provider to ensure continuity of care.
Horizon Family medicine will continue to provide care for infants in the newborn nurseries at both Johnston Health – Clayton and Johnston Health – Smithfield.
We look forward to continuing to provide you with high quality care during your weekday office visits at any of our Horizon locations and/or at our weekend urgent care offices in Smithfield and Clayton.
